HOW TO JOIN 7-700-7 LivCCCoop

TO JOIN, do visit our Coop office at Rm 216 Bayanihan Building, Otek St., Baguio City and bring the following:

1. two 2x2 pictures 2. proof of income [Employment Certificate for those employed; Business Permits for those with businesses OR ask for our Coop's Business Profile Form and fill it up] 3. barangay certificate 4. sketch of residence 5. cash of Php1,000.00 for membership with both the Coop [Php200.00} and with the Seven Seven Hundred Seven Association of Baguio City [Php500.00] and for your initial share capital with the Coop [Php300.00]

Request for the Cooperative and Association Membership Forms and fill them up attaching above documents then pay the required membership fees and initial share capital.

Please take note that our Coop is associational in membership, that is, only members of the Association can become coop members and so the necessity of being a member to the Association as well.

On the ID Form, affix your specimen signature for ID and write legibly your complete name [including full middle name] and your complete address.

Do attend Coop PMES [pre-membership educational seminar] every third Saturday of the month, 1:00 to 3pm at the Coop Office. A minimal PMES fee of Php50.00 shall be charged for the PMES certificate.
